Attention

Contractors MUST contact us to select/confirm payment terms and receive an RA number prior to sending any units in for repair. Any units received without having done this WILL be refused and returned to sender.


Do NOT send units for repair via the US Postal service. We do not have mail delivered to the repair center address. Send via parcel carrier (ie.,UPS,Airborne,Fed Ex etc.)


At this time this form will not work with Netscape Web browsers. If you only have access to Netscape call the repair center at the number below to receive verbal instructions on returning units for repair. Sorry for the inconvenience.

Repair center email is checked at the minimum 3 times daily Monday thru Friday. At 8:30 am,12:00 noon and 4:30 pm (all times Pacific Standard Time).

Important: If you are sending a terminal in for VTS Software Update only, please do not send in anything but your Palm Terminal. Sending extra equipment will only increase shipping and handling charges. Thank you.


If you are Not a Verizon employee, but a contractor doing work for Verizon use the Contractor Repair Authorization Form.

If you Are a Verizon employee use the Verizon Repair Authorization Form.

If you Are an ALLTEL employee or contractor use the ALLTEL Repair Authorization Form.
